Case Management

Our case management agency specializes in providing comprehensive case management services to individuals with substance use issues. Our case managers work closely with clients to assess their needs, develop individualized treatment plans, and coordinate access to a wide range of services, including substance use treatment, mental health services, housing and employment assistance, and more. We understand that recovery is a journey and our goal is to support our clients every step of the way and empower them to reach their goals. We also provide support and education to families of clients to help them understand and navigate the recovery process

Our dedicated case managers work closely with clients to assess their needs, develop individualized treatment plans and coordinate access to a wide range of evidence-based services, such as detox, counseling, therapy and aftercare support.
